certain responsibilities and roles:

  • Assist regional team in ensuring that all PLHIV identified in facilities and communities are connected to ART and other pertinent services.
  • Manage the implementation of protocols for HIV testing services, prevention, and linkages while offering technical support to HCPs, peer educators (PE), and other actors.
  • Create and promote creative approaches to lower treatment breaks among PLHIV in supported sites.
  • Work with the HIV prevention & linkages and QI teams to do recurring assessments of the standards of care for HIV prevention & linkages in high volume THPS sponsored sites.
  • providing HCPs and community-based NGOs with pertinent job aids and resources related to HIV prevention.
  • Utilizing mentorship techniques and abilities, provide supportive supervision and mentoring to HCPs, PEs, LCs, and sub-granted NGOs in partnership with team members in order to increase the quality of services.
  • Review the quarterly plans, reports, and progress of the HIV prevention & connections sub-grantees in the district.
  • In order to undertake HIV prevention and linkages activities for both facility and community settings, maintain contact with and assist R/CHMT, and subgranted community NGOs.
  • Participate in regional discussions on HIV prevention & connections on behalf of THPS.
  • Increase cross-sectoral cooperation with the Regional, District, and Council HIV and AIDS Coordinators (R/DACC/(CHACC) and other community partners.
  • Provide technical assistance for using national appointment and tracking systems, which provide tools for locating and tracing patients who don't show up for appointments or treatments.
  • Implement creative strategies to increase customer follow-up, referrals, and appointment adherence in the
  • locating individuals who have had treatment interruptions and the community.
  • Create viremia clinics for patients who aren't on medications by working with the clinical teams.
  • Contribute to the creation of the R/HIV CHMT's annual work plans.
  • Before submitting, review and enter each quarterly, semi-annual, and yearly progress report on HIV prevention and connections.
  • Prepare recurring technical reports, best practices, and weekly and monthly reporting to track progress.

Required Knowledge, Skills, and Experience:

  • It is required to have a degree in nursing, sociology, or medicine.
  • Candidates who have completed relevant postgraduate training in fields like medicine, sociology, nursing, etc. would have an edge.
  • three years or more working in the field of HIV Testing, Prevention, and Linkages
  • knowledge of cooperating with NGOs and local government partners (R/CHMTS).
  • The capacity to interact diplomatically and effectively with a variety of government agencies, nongovernmental groups, faith-based organizations, and the business sector.
  • demonstrated interpersonal abilities for contributing successfully to a team.
  • autonomous work capability and superior problem-solving abilities.
  • English and Swahili proficiency
  • computer expertise
